What is Excelsior Pass Plus?

Excelsior Pass Plus is a secure, digital copy of your COVID-19 vaccination record or negative test results. 

Your Excelsior Pass Plus provides safe access to your COVID-19 vaccination record or negative test results. The inclusion of health information enables you to have a verifiable record of your COVID-19 vaccination or negative test result at your fingertips. It includes the same information you would find on your CDC Vaccination Card or paper laboratory test results for record-keeping and/or usage outside of New York State.

Excelsior Pass Plus can be accessed and stored through the New York State Wallet app, or through the Excelsior Pass Web Portal and printed. The New York State Wallet app can be downloaded at no cost from the Apple App Store and Google Play Store.

Excelsior Pass Plus is available in more than 10 languages, including: English, Spanish, Chinese (traditional and simplified), Russian, Haitian-Creole, Korean, Bengali, Arabic, Italian, Polish, and Yiddish.

 

Are there different Excelsior Pass Plus types?

There are currently three Excelsior Pass Plus types:

  • Excelsior Vaccination Pass Plus (available 15 days after the final dose of the vaccine was administered; as a digital copy of an individual vaccination record, there is no expiration date)
  • Excelsior PCR Pass Plus (valid until midnight on the third day after a test)
  • Excelsior Antigen Pass Plus (valid for 6 hours from the time of a test)

 

I received my booster/additional COVID-19 vaccine dose. How do I get this information on my Excelsior Pass Plus?

If you received your COVID-19 booster/additional dose outside of NYC, wait at least 3 – 4 days, then simply visit epass.ny.gov to retrieve a new Excelsior Vaccination Pass Plus at any time, which will now have this information included.

If you received your COVID-19 booster/additional dose in NYC, wait at least 7 days, then simply visit epass.ny.gov to retrieve a new Excelsior Vaccination Pass Plus at any time, which will now have this information included.

Retrieving an Excelsior Pass Plus

How do I know if I am eligible to retrieve an Excelsior Vaccination Pass Plus?

You are eligible to retrieve your Excelsior Vaccination Pass Plus if:

  • It has been 15 days or longer since you received the final dose in your vaccine series, you have not tested positive for COVID-19 in the last 10 days, and:
    • You have been fully vaccinated in the State of New York OR
    • You are a New York resident who has been fully vaccinated in the State of New Jersey or, Vermont OR
    • You are a New York resident and your New York State healthcare provider has entered your out-of-state COVID-19 vaccination information into the secure New York State and/or New York City immunization databases.

 

Please note that you must have received one of the COVID-19 vaccines approved or authorized by the Food and Drug Administration (FDA) in the United States in order to retrieve your Excelsior Vaccination Pass Plus. At this time, this includes the Pfizer-BioNTech, Moderna, and Janssen/Johnson & Johnson COVID-19 vaccines. Find more information on the COVID-19 vaccines. New Yorkers who received a COVID-19 booster/additional dose at least 3 – 4 days ago, can simply visit epass.ny.gov to retrieve a new Excelsior Vaccination Pass Plus at any time, which will now have this information included.

 

If I was vaccinated outside of New York State but meet the eligibility requirements for an Excelsior Vaccination Pass Plus, how do I retrieve my Excelsior Vaccination Pass Plus?

For New York State residents who were fully vaccinated outside of New York State, New Jersey or Vermont, your New York healthcare provider must input your vaccination information into the secure New York State and/or New York City immunization databases, and that provider must be administering the COVID-19 vaccine. Please note that when you attempt to retrieve your Excelsior Vaccination Pass Plus from the Excelsior Pass Web Portal, you will need to enter the county where your New York healthcare provider is located, NOT the county where you received your COVID-19 vaccine.

 

How do I know if I am eligible to retrieve an Excelsior PCR Pass Plus?

You are eligible to retrieve your Excelsior PCR Pass Plus if your PCR test was conducted in the State of New York, administered within 3 days of attempting to retrieve your Excelsior PCR Pass Plus, and your results came back negative. An Excelsior PCR Pass Plus is typically available within a few hours of receiving your negative test results from your testing laboratory.

 

How do I know if I am eligible to retrieve an Excelsior Antigen Pass Plus?

You are eligible to retrieve your Excelsior Antigen Pass Plus if your antigen test was conducted in the State of New York, administered within 6 hours of attempting to retrieve your Excelsior Antigen Pass Plus, and your results came back negative. An Excelsior Antigen Pass Plus is typically available within a few hours of receiving your negative test results from your testing laboratory.

 

I have children under 18 years old. Do they need an Excelsior Pass Plus? How can they get an Excelsior Pass Plus?

Anyone who has received a COVID-19 vaccine or negative COVID-19 test results in the State of New York is eligible for an Excelsior Pass Plus, including children under 18 years old. Parents and legal guardians may retrieve and store Passes on behalf of children or minors under legal guardianship.

 

What information do I need to provide to retrieve my Excelsior Pass Plus?

To retrieve your Excelsior Pass Plus, you will need to provide basic personal information, including your first and last name, date of birth, ZIP code, and phone number. You will also need to verify your information with correctly answering three "challenge questions", which are unique to you. Excelsior Pass Plus also requires a valid phone number or email for two-factor authentication, which will enable us to match this information to your health record.


Can I update my Excelsior Pass Plus to include my middle name?

Yes, you can retrieve a new Excelsior Vaccination Pass Plus with your middle name. Simply visit epass.ny.gov, which now has a field for you to enter this information. Since your Excelsior Vaccination Pass Plus is a secure, digital copy of your vaccination record, you must also delete your current Excelsior Vaccination Pass Plus so you can save your new record with your middle name included.

New Yorkers should know that while they can only hold one Excelsior Vaccination Pass Plus (per individual) at a time, they can delete or retrieve a new Excelsior Pass Plus at any time. Parents and legal guardians may retrieve, and store Passes on behalf of children or minors under legal guardianship.

How is my Excelsior Pass Plus generated?

Your Excelsior Pass Plus is generated based on data provided by your vaccine administrator or testing laboratory to the secure New York State and/or New York City immunization and COVID-19 testing databases.

If you are a New York State resident and were vaccinated at a Veterans Affairs location within New York State, you are now eligible to retrieve an Excelsior Vaccination Pass. However, some entities that are not under the regulatory authority of the State of New York (e.g., federal entities, first nations, and jurisdictions outside of New York State), may not report into these systems, which may make an Excelsior Pass unavailable at this time.


Can I add my Excelsior Pass Plus to my Apple Wallet?

Yes, you can add an Excelsior Pass Plus to your Apple Wallet by following a few quick steps:

  • First, ensure your iOS device is using an iOS 15 operating system. If it is not, you will need to update your operating system to iOS 15 before proceeding. 
  • Next, you will need to scan the QR code within your Excelsior Pass Plus. This can be done by downloading your Excelsior Pass Plus on another mobile device or a computer:
    • Open the Camera app from the Home Screen, Control Center, or Lock Screen on your iPhone or iPod touch
    • Select the rear-facing camera
    • Hold your device so that the QR code appears in the viewfinder in the Camera app. Your device recognizes the QR code and shows a Health app notification
    • Tap the Health app notification
  • Next, a pop-up for your Apple Wallet will appear on your smartphone device. You will then tap the “Add to Wallet and Health” button to add your Excelsior Pass Plus—a verifiable vaccination record—to the Wallet and Health apps.
  • Finally, tap “Done”.
     

What should I do if I am not able to retrieve an Excelsior Pass Plus?

First, be sure to confirm that you are eligible to retrieve an Excelsior Pass PlusIf you are still having trouble after confirming eligibility, please make sure that the information you are entering matches the information (e.g., name, date of birth, and ZIP code) from your CDC Vaccination Card or test results, as you will need to ensure an accurate match to retrieve your Excelsior Pass Plus. You will also need to match your phone number or email address to the information associated with your vaccine record or negative test results.

 

I am still unable to retrieve my Excelsior Pass Plus, what should I do?

If you are still unable to retrieve your Excelsior Pass Plus, please reach out to the Excelsior Pass Help Desk at (844) 699-7277 to receive support.

You are also encouraged to reach out to your vaccine administrator or testing laboratory to ensure they have entered your phone number or email address correctly, as you will need to match that information accurately to retrieve your Excelsior Pass Plus. Please know that per New York State Department of Health (NYSDOH) guidance, all New York State vaccine administrators must submit vaccination data to the secure New York State and/or New York City immunization and COVID-19 testing databases and have staff available to both review and correct the data they input, if data entry issues are identified.

Using Excelsior Pass Plus

How do I show my Excelsior Pass Plus to participating organizations?

You can present your Excelsior Pass Plus on your smartphone, through the New York State Wallet app, or you can print your Excelsior Pass Plus from the Excelsior Pass Web Portal and present the printed version.

Organizations who accept Excelsior Pass Plus can verify your Pass with the NYS Scanner app.

If you are an adult aged 18+, you will also need to present a valid photo ID with name and date of birth alongside each unique Excelsior Pass Plus. Parents and legal guardians may retrieve and store Passes on behalf of children or minors under legal guardianship.

 

Who can accept my Excelsior Pass Plus?

Your Excelsior Pass Plus may be accepted by participating organizations in New York State seeking proof of vaccination and/or negative test results, and/or by entities outside of New York State who may accept SMART Health Cards developed by VCI, which are already in use in states such as California, Louisiana, Virginia, and Hawaii.

 

What is the SMART Health Cards Framework, developed by VCI?

Excelsior Pass Plus is built using the SMART Health Cards Framework and specifications, developed in partnership with VCIVCI is a voluntary coalition of 800 public and private organizations committed to empowering individuals by providing access to a trustworthy and verifiable copy of their vaccination records or negative test results in digital or a CDC Vaccination Card using open, interoperable standards.

Excelsior Pass Plus expands on New York State's commitment to empower individuals with access to a free, trustworthy, privacy-preserving, and verifiable copy of their own vaccination records or lab results in paper or digital form. Excelsior Pass Plus supports the harmonization of standards needed for the issuance and acceptance of verifiable health credentials worldwide — signed clinical data bound to an individual identity.

 

Am I required to use Excelsior Pass Plus?

No, participation is voluntary. Your Excelsior Pass Plus empowers you to be in control of your health information. Even after retrieving an Excelsior Pass Plus, you decide if, when, or how to use and show it. Where applicable, alternate proof of vaccination and/or negative test results, such as a CDC Vaccination card or paper test results, must also be accepted.

 

How much does it cost to use Excelsior Pass Plus?

Excelsior Pass Plus is free.

 

Can I show more than one type of Pass on the same device?

Yes. You may retrieve, store, or show as many Passes as you may be eligible for. This includes:

  • Excelsior Vaccination Pass Plus
  • Excelsior PCR Pass Plus
  • Excelsior Antigen Pass Plus

 

Can I show Excelsior Pass Plus for more than one individual on the same device?

Yes, multiple Excelsior Pass Plus Passes for adults aged 18+ and children under 18 can be stored on the same device.

 

How long will I have to use my Excelsior Pass Plus? Will it expire?

Your Excelsior Vaccination Pass Plus is a digital copy of your COVID-19 vaccination record. As such, your Excelsior Vaccination Pass Plus has no expiration date.

Your Excelsior PCR Pass Plus is valid until midnight on the third day after a test. After your Excelsior PCR Pass Plus expires, a new negative PCR test is required to retrieve a new Excelsior PCR Pass Plus.

Your Excelsior Antigen Pass Plus is valid for 6 hours from the time of a test. After your Excelsior Antigen Pass Plus expires, a new negative Antigen test is required to retrieve a newExcelsior Antigen Pass Plus.

 

Can I get a refund if I bought a ticket to an event, but my Excelsior Pass Plus is not valid?

Refund policies are unique to the organization you are visiting. It is recommended that you consult with the organization directly as to their refund policies.

 

The name of my app changed to “New York State Wallet.” What does this mean, and will it impact the credential I currently have (Excelsior Pass Plus)?

The change of the app name will have no impact on your existing Excelsior Pass Plus. The upgraded New York State Wallet provides safe and secure access to your State-issued digital passes, licenses, and records through your mobile device.

How can I retrieve an Excelsior Pass Plus if I don’t have access to a smartphone?

Participation in Excelsior Pass Plus is voluntary and designed to be inclusive of all New Yorkers, regardless of access to a smartphone. You can easily print your Excelsior Pass Plus from the Excelsior Pass Web Portal.

You can also show alternate proof of COVID-19 vaccination or negative test results, such as a CDC Vaccination card or paper test results, directly at an organization.

 

Is Excelsior Pass Plus available in multiple languages?

Yes. The application has undergone accessibility testing to ensure it is easy to use for those who may have hearing or vision impairments.

What is the difference between a PCR test and an antigen test?

According to the Food and Drug Administration (FDA), PCR tests are molecular tests that detect the virus’ genetic materials, and antigen tests detect specific proteins from the virus. You can read more about testing in the FDA’s Coronavirus Disease 2019 Testing Basics.

 

Where can I get tested for COVID-19?

Testing for COVID-19 is widely available throughout New York State. Individuals who have questions about COVID-19 testing should call the New York State COVID-19 Hotline at 1-888-364-3065 or visit NYS Find A Test Site Near You.

 

If I tested positive for COVID-19 antibodies, can I retrieve an Excelsior Pass Plus?

At this time, Excelsior Pass Plus is only available for presenting COVID-19 vaccination information or negative COVID-19 diagnostic test results in accordance with New York State Department of Health (NYSDOH) guidance.

Security and Privacy

Is my data private and secure?

Yes. Your data is private, secure, and only used to retrieve your Excelsior Pass Plus. Additionally:

  • Your personal data will not be used for sales or marketing purposes or shared with a third party, other than for the sole purpose of validating your COVID-19 vaccination or negative test results.
  • Your Excelsior Pass Plus contains cryptographic signatures to prove to verifiers that it is a genuine credential issued by New York State and has not been tampered with or altered.
  • You must also present a valid photo ID with your name and date of birth alongside your Excelsior Pass Plus to ensure that your Pass belongs to you.

 

You choose when, if, and how to present your Excelsior Pass PlusPresenting your Excelsior Pass Plus is just like sharing your CDC Vaccination Card or paper test results.

 

Will the New York State Wallet app update impact my Excelsior Pass Plus?

No. Your Excelsior Pass Plus will not be impacted by the Wallet upgrade. The upgrade will provide more security for you by having the option to create and utilize a PIN and/or the FaceID/TouchID feature for iOS users or the Facial Recognition/Fingerprints feature for Android users.

 

How do I setup FaceID/TouchID or Facial Recognition/Fingerprints?

FaceID/TouchID or Facial Recognition/Fingerprints are optional to setup and use in the New York State Wallet. FaceID/TouchID must be setup within your iOS device first to be utilized for the New York State Wallet app. Facial Recognition/Fingerprints must be setup within your Android first to be utilized for the New York State Wallet app. Once this is complete, you will be given the option to use these features within the New York State Wallet app after setting up your PIN.

For iOS users whose mobile devices do not support FaceID/TouchID or do not have this feature enabled, you will only be asked to set up a PIN. For Android users whose mobile devices do not support Facial Recognition/Fingerprints or do not have this feature enabled, you will only be asked to set up a PIN.

 

Do I have to create a PIN to use my New York State Wallet app/Excelsior Pass Plus?

Users have the option to upgrade the New York State Wallet app to the latest version. Once updated, users will be required to setup a PIN. Users will also have the option to utilize FaceID/TouchID feature for iOS users and Facial Recognition/Fingerprints for Android users. This has been added as an enhanced security feature.

 

Why am I being asked to create a PIN and/or use FaceID/TouchID feature for iOS users or Facial Recognition/Fingerprints for Android users?

New York State's top priority is to ensure your personal data is safe and secure. Like other apps that use FaceID/TouchID/Facial Recognition/Fingerprints, these features provide users the option to further enhance the security of their data beyond the PIN and provide quick and easy access to the Wallet.

 

How do I create a PIN?

To create a PIN:

  1. Download, or update to, the latest version of the New York State Wallet app.
  2. Then, once you open the app, you will be prompted to set an 8-digit PIN.
    a. The PIN cannot have more than three repeating numbers (for example, 111) and cannot have more than three consecutive numbers (for example, 123).

 

I forgot my PIN, what do I do?

To reset your PIN:

  1. Open the New York State Wallet app.
  2. On the screen that asks for your PIN, select “Forgot your PIN”.
  3. Then, follow the prompts.

To protect your data, resetting your PIN will result in removal of your passes from your Wallet app. Tap ‘Get My Pass’ or ‘Add My Pass’ to securely add your passes, licenses, and records back into your Wallet. 

 

What is two-factor authentication, and how does it keep my information secure when retrieving an Excelsior Pass Plus?

In addition to entering your first and last name, date of birth, ZIP code, and phone number, you must correctly answer a set of “challenge questions” unique to you (for example, ‘When did you receive the final dose of your vaccination series?’). Answers to the “challenge questions” are used to verify your identity and will not be stored.

You will also need to enter a randomly generated 6-digit code to obtain your Excelsior Pass Plus. This code will be securely delivered as a text to a cell phone number; a voice message to a landline telephone number; or an email to an email address associated with your COVID-19 vaccination record. This additional authentication factor helps to further reduce the opportunity that someone else other than you can obtain your COVID-19 vaccination record or test results.

 

Will my personal information be stored in the New York State Wallet app?

If you choose to use the New York State Wallet app to store your Excelsior Pass Plus, your first name, last name, and date of birth will be stored on your Excelsior Pass Plus. You choose when, if, and how to present your Excelsior Pass Plus.

For an Excelsior Vaccination Pass Plus, data matching the information provided by your CDC Vaccination Card will also be stored. This includes:

  • Date(s) of vaccine dose(s)
  • Vaccine type
  • Vaccination site
  • Lot number

 

For an Excelsior PCR Pass Plus or Excelsior Antigen Pass Plus, data matching information provided by your test results will also be stored. This includes:

  • Test date and time
  • Test type
  • Test result
  • Testing site
  • LOINC Code
  • Test descriptor

 

Answers to the “challenge questions” used to verify your identity to retrieve your Pass will not be stored. 

You may delete an Excelsior Pass Plus at any time.

 

What information is visible on my Excelsior Pass Plus?

As a secure digital copy of your vaccination record, your Excelsior Vaccination Pass Plus includes the same information on your CDC Vaccination Card. This includes:

  • Date(s) of vaccine dose(s)
  • Vaccine type
  • Vaccination site
  • Lot number

 

Your Excelsior PCR Pass Plus or Excelsior Antigen Pass Plus includes the same information on your paper test results. This includes:

  • Test date and time
  • Test type
  • Test result
  • Testing site
  • LOINC Code
  • Test descriptor

 

You can ask verifying organizations that will scan the QR code in your Excelsior Pass Plus how they will use your data. Presenting your Excelsior Pass Plus is just like sharing your CDC Vaccination Card or paper test results. Only you choose when, if, and how to present your Excelsior Pass Plus.

 

What information can an organization see when they scan my Excelsior Pass Plus through the NYS Scanner app?

When an organization uses the NYS Scanner app to scan your Excelsior Pass Plus, the only personal information they will be able to see is your first name, last name, and date of birth.

If the Scanner app scans a valid pass, the Scanner app will display “Pass Valid.” If the Scanner app scans a pass that does not meet the State’s standards of issuance, the Scanner app will display “Pass Not Valid.“

 

Will organizations using the NYS Scanner app to scan my Excelsior Pass Plus be able to save or store my personal information?

No. Organizations using the NYS Scanner app to scan your Excelsior Pass Plus will not be able to save or store your information. This is why an individual must re-present their Pass each time they enter an organization, business, or venue.

It is recommended you only share your Excelsior Pass Plus with known and trusted parties. Prior to presenting an Excelsior Pass Plus to an organization, it is recommended you request and review the verifier’s policy on collecting, storing, and sharing personal information to confirm that the information obtained from scanning your Excelsior Pass Plus will only be used for the purpose of validating your COVID-19 vaccination or test information at that time and then immediately deleted.

If you choose to print and use a paper copy of your Excelsior Pass Plusyou should secure and maintain physical control over your printed Excelsior Pass Plus just as you would with your CDC Vaccination Card, test results, or other sensitive documents containing sensitive, personal health information.

Your Excelsior Vaccination Pass Plus shows the same information as your CDC vaccination card. Your Excelsior PCR Pass Plus and/or Excelsior Antigen Pass Plus shows the same information as your COVID-19 test results.

 

Does Excelsior Pass Plus track my location?

No. Excelsior Pass Plus does not track your location.

 

Will New York State or anyone else be notified if I attempt to retrieve an Excelsior Pass Plus after a positive COVID-19 test result?

No. Excelsior Pass Plus does not share your data with anyone. Additionally, an Excelsior Pass Plus cannot be retrieved if you receive a positive test result.

 

Can I use this app to see if someone else has been fully vaccinated or tested negative for COVID-19?

No. Excelsior Pass Plus is intended for your own personal use only.

What are the basic requirements for using Excelsior Pass applications?

Excelsior Pass applications are compatible with smartphones running iOS 13.0+ or Android 7.0+.

If preferred, you may also print a paper copy of your Excelsior Pass Plus from the Excelsior Pass website or New York State Wallet app.

 

What should I do if my vaccination or test information cannot be confirmed?

For an Excelsior Vaccination Pass PlusPlease check that you have entered your information exactly as it appears on your CDC Vaccination Card, as you will need to accurately match that information to retrieve your Excelsior Pass Plus. This includes:

  • First and last name
  • Date of birth
  • The county where you received your vaccine (for New Yorkers vaccinated outside of New York State, New Jersey or Vermont, this is the county in which your New York healthcare provider who is entering your information is located)
  • The date you received the last dose of your vaccine series (one dose of Janssen/Johnson & Johnson (J&J)  vaccination series or the second of the two-dose Pfizer or Moderna series)
  • The type of vaccine you received

 

For an Excelsior PCR Pass Plus or Excelsior Antigen Pass PlusPlease check that you entered your information exactly as it appears on your test results, as you will need to accurately match that information to retrieve your Excelsior Pass Plus. This includes: your first name, last name, the location where you received your test, the date you were tested, and the type of test you received.

A two-factor authentication must be performed to retrieve your Excelsior Pass Plus. In order to perform this, a valid email address or phone number must be on record.

If you still can’t get your Excelsior Pass Plus, you can:

  • Contact your vaccine administrator to confirm your correct information was submitted. All vaccine administrators must have staff dedicated and available to correct these issues. Once a testing laboratory has submitted test result information to the State, it cannot be modified.
  • If you were vaccinated at a New York City-run site and require a change to your vaccination record, you can call 311 from within the City or 212-639-9675 from outside New York City or email [email protected]. Additionally, if you are a New York City resident and fully vaccinated but lost your CDC Vaccination Card, you can request a copy of your COVID-19 vaccination record by calling 311.
  • Call the Excelsior Pass Help Desk at (844) 699-7277 for general inquiries.

 

In the meantime, you can always show alternate proof of COVID-19 vaccination or negative test results, like a CDC Vaccination card or paper test results, directly at an organization.

As a provider, what COVID-19 vaccination data am I responsible for providing?

You are responsible for reporting complete and accurate COVID-19 vaccination data to the appropriate data system (CIR for New York City providers and NYSIIS for all other providers) within 24 hours of vaccine administration. All required and critical fields must be completed at the time the data is entered. As someone who administers COVID-19 vaccines, what do I do when customers/patients tell me they are having trouble retrieving an Excelsior Pass Plus?

The provider who administered the vaccine is responsible for entering all vaccine data correctly into NYSIIS (or CIR if the provider is located in New York City). If one of your customers/patients is having trouble retrieving an Excelsior Pass Plus, your first action should be to check their information in NYSIIS/CIR (don’t just look in your system/electronic medical record). If I am a New York State healthcare provider, can I submit COVID-19 vaccination information for patients who are residents of New York State, but received their vaccine outside of the State?

If you have patients who were vaccinated outside of New York State who can show proof of vaccination, you should enter those patients’ immunization records into the secure New York State and/or New York City immunization databases. You will submit these as historical doses to reflect that you did not administer the dose to the patient.
